You can print out copies of our notification forms by clicking on the links below:
SWCAR Notification / Warning Card (140K)
To report antenatally detected anomalies. Please include EDD and NHS numbers.
SWCAR Data Form (50K)
To report or confirm anomalies noted at birth or in the neonatal period. You will need maternal details to complete this form.
Paediatric Reporting Form (90K)
To report anomalies when maternal details are not known.

To make sure the labels fit on the page when you print, look for this setting in the dialog box: Page scaling (under Page handling) set it to 'None'.

What anomalies to report
Not sure what to report? In general, any anomaly or disorder present before or at birth should be reported to SWCAR. Some minor anomalies do not need to be notified. For more details print out our inclusion/exclusion lists by clicking on the links below:

We code all anomalies using the World Health Organisation WHO ICD-10 Intenational Classification of Diseases. Most, but not all, anomalies held on the register are Q codes included in Chapter XV11. For more details of this coding system click on the WHO link below:
